Enrollment (CEC)
Enrollment at St. Augustine Health Ministries Child Enrichment Center (CEC) is open to children from six weeks to five years old, regardless of race, gender, religion, or national origin. St. Augustine Health Ministries Child Enrichment Center accepts county vouchers. Enrolled children also enjoy nutritious daily meals and snacks, planned and prepared by St. Augustine’s Food Service staff.

During the interview, you will receive a print copy of our policies and materials to help you decide if our center is right for you and your child. We encourage you to bring your child with you so everyone can meet the Child Enrichment Center family and see what opportunities are here at the St. Augustine Child Enrichment Center! This personal connection will help you decide if our center is a match for you and your child. - Fill out the Enrollment Application.
To reserve an available space, please download and return the completed enrollment application; include the $30 non-refundable registration fee. This will secure a space for your child in our center. We will work with you and the teachers to select a start date for your child. If you have any questions/concerns, please contact 216.939.7681 or email in**@st****.org . - Child Care Forms
In addition to the enrollment application, we will need the following forms and a copy of your child’s immunization record and physical form for your child’s pediatrician to be returned on or before your child’s first day of attendance.
— CEC Physical Well Child Exam Form: Your child’s pediatrician will fill this out.
— ODJFS Medical Physical Care Plan Form: Your child’s pediatrician will fill this out.
