Job Summary
We are seeking a compassionate and dedicated PRN RN-Community Hospice Nurse to provide high-quality care to patients in our hospice facilities. The ideal candidate will be responsible for delivering comprehensive nursing care, ensuring comfort, and supporting patients and their families during end-of-life transitions. This role requires strong clinical skills, empathy, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities
Assess patient needs and develop individualized care plans based on their medical history and preferences.
Administer medications and treatments as prescribed, including managing pain relief and symptom control.
Monitor vital signs and perform basic life support as needed to ensure patient safety.
Provide emotional support and education to patients and families regarding the hospice process and available resources.
Coordinate with physicians, social workers, and other healthcare professionals to facilitate comprehensive care.
Document patient progress and changes in condition accurately in medical records.
Assist with airway management and other critical interventions when necessary.
Participate in triage for urgent patient needs, ensuring timely responses to changing health conditions.
Engage in home care visits, providing compassionate support in a familiar environment for patients.
Skills
Proficient in nursing skills related to long-term care, including experience in nursing homes or similar settings.
Knowledge of coding practices for accurate documentation of patient care activities.
Ability to dialyze patients when required, demonstrating advanced clinical capabilities.
Strong understanding of vital signs monitoring and basic life support techniques.
Excellent communication skills for effective interaction with patients, families, and healthcare teams.
Empathy and compassion for patients facing end-of-life challenges, fostering a supportive environment.
Ability to manage complex patient situations with critical thinking and problem-solving skills.
