Overview
We are seeking a compassionate and dedicated PRN Licensed Practical Nurse (LPN) to join our Home Health Care team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings. As an LPN, you will play a crucial role in supporting registered nurses and physicians while ensuring the comfort and well-being of patients.
Duties
Provide direct patient care, including administering medications and treatments as prescribed.
Conduct patient assessments and monitor vital signs.
Document patient information accurately in electronic health records (EHR) systems.
Collaborate with healthcare team members to develop and implement individualized care plans.
Educate patients and families on health management and disease prevention.
Maintain a clean and safe environment for patients.
Participate in discharge planning and case management activities.
Support patients with activities of daily living (ADLs) such as bathing, dressing, and feeding.
Skills
Candidates should possess a combination of required and preferred skills, including but not limited to:
Experience in critical care, long-term care, assisted living, or home care settings.
Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and CPT coding.
Familiarity with acute pain management, geriatrics, dementia care, and Alzheimer’s care.
Skills in IV infusion, catheterization, medication administration, and specimen collection/processing.
Ability to perform physical examinations and triage patients effectively.
Strong understanding of infection control practices and aseptic techniques.
