Job Overview
We are seeking a compassionate and dedicated PRN RN On-Call Community Hospice Nurse to join our team. The ideal candidate will provide high-quality, patient-centered care to individuals with terminal illnesses, ensuring comfort and dignity during their final stages of life. This role requires a strong commitment to empathetic care, effective communication, and collaboration with interdisciplinary teams to support patients and their families.
Duties
Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs.
Develop and implement individualized care plans in collaboration with the healthcare team.
Administer medications and treatments as prescribed, including Botulinum toxin treatment when applicable.
Provide triage support for patients experiencing acute symptoms or emergencies.
Assist with laboratory tests and medical imaging as necessary.
Facilitate specimen collection processing to ensure accurate diagnostics.
Offer specialized care for patients with dementia and geriatrics, focusing on comfort and quality of life.
Educate patients and families about end-of-life care options, symptom management, and available resources.
Maintain accurate documentation of patient care activities in compliance with regulatory standards.
Skills
Proficiency in MDS (Minimum Data Set) assessments.
Strong background in emergency medicine to respond effectively to urgent patient needs.
Experience in laboratory procedures and medical imaging techniques.
Knowledge of dementia care practices tailored to individual patient requirements.
Ability to perform specimen collection processing accurately.
Familiarity with toddler care principles may be beneficial in family-oriented settings.
Excellent communication skills for interacting with patients, families, and healthcare professionals.
Strong organizational skills to manage multiple patient cases efficiently.
